I discovered an error in my VA medical records. The error is in this note (printed copy also enclosed):

Here is the part I would like corrected: "[print or type the mistake]".

Please correct to: “[print or type how it should have been written.]”

I understand that I have the right to request an amendment of records pertaining to me,{footnote 1}  and that you will notify me within 10 business days that you have received my request.{footnote 2}
